Add support for Monument Bank (UK/GoCardless)

Please add Monument Bank to the list of supported UK institutions. Monument Bank has an active Open Banking API that is supported by GoCardless (one of Lunch Flow's upstream providers), but it is not currently selectable when setting up a new connection in Lunch Flow.
